Clarity were invited by the Managing Director of North Hampshire CCG and Director of Commissioning for West Hampshire CCG to conduct a full review of patient escalation and flow processes across the Hampshire Hospitals NHS Foundation Trust and its system partners.

Taking place from August-September ’18, the review focussed on two new processes: a daily multi-agency patient review meeting, and a weekly service level escalation meeting, and also on the impact of a recently-adopted remedial emergency department action plan ‘RemED’. Acting as a critical friend, we reviewed plans and progress against their deliverables.

We observed meetings, provided structural improvement suggestions, giving real-time feedback and staff coaching to improve processes. Further discussions were had with key stakeholders across the system to understand the wider impact of changes and to identify additional improvements.

During visits we emphasised the importance of optimising all stages of the patient journey from ‘home to home’, and highlighted areas of potential high impact to improve system resilience.

Our final project report contained a full review of patient escalation and flow processes across the Trust, with resulting recommendations.

A full review of patient escalation and flow processes was conducted, and a report delivered, with recommendations for a series of high-impact follow-up actions.
