  • Clarity were invited by the Managing Director of North Hampshire CCG and Director of Commissioning for West Hampshire CCG to conduct a full review of patient escalation and flow processes across the Hampshire Hospitals NHS Foundation Trust and its system partners.

  • Taking place from August-September ’18, the review focussed on two new processes: a daily multi-agency patient review meeting, and a weekly service level escalation meeting, and also on the impact of a recently-adopted remedial emergency department action plan ‘RemED’. Acting as a critical friend, we reviewed plans and progress against their deliverables.

Clarity Approach

  • We observed meetings, provided structural improvement suggestions, giving real-time feedback and staff coaching to improve processes. Further discussions were had with key stakeholders across the system to understand the wider impact of changes and to identify additional improvements.

  • During visits we emphasised the importance of optimising all stages of the patient journey from ‘home to home’, and highlighted areas of potential high impact to improve system resilience.

  • Our final project report contained a full review of patient escalation and flow processes across the Trust, with resulting recommendations.
